| The D2 form is where to start your application, renewal or upgrade. You can apply online for the form. It can only be ordered to a UK postal address. This form asks for your details and needs to know exactly what you are applying for. It also asks you to disclose basic medical information and whether you expect to pass the visual standards.
The D2 form is where your licence upgrade starts and is posted with the D4 medical form. It can be downloaded below:
