| The D4 form was last updated in Feb 2023. The form is green and slightly different to the previous pink D4 form. The grey sections are to be filled in by the applicant: front cover and footer of every page. The first page is the visual assessment and can be done by any doctor,optician or optometrist, provided they have the correct DVLA eye chart. The D4 form can be downloaded and printed on your own printer. Do not sign the form on the last page until you are at the appointment. The signature on the D4 Form must be witnessed by the doctor.
