Last updated: 2 November 2026

Book your HGV / D4 medical with Dr Stephen R Walton
Sheffield S12 -- £80 for 30 minute GP examination

According to GOV.UK, to become an HGV driver you need a full car licence, to be over 18 (there are some exceptions), and to gain the Driver Certificate of Professional Competence (CPC).

Step 1: Get your D4 medical

You need a medical, recorded on the DVLA D4 form, when you apply for your provisional lorry entitlement. Our GP medical takes 30 minutes and includes the eye test.

Step 2: Apply for a provisional HGV licence

Send your provisional application to DVLA with your completed D4. Find out which category you need in our licence categories guide.

Step 3: Pass the Driver CPC tests

GOV.UK says there are 5 tests that make up the full Driver CPC:

TestWhat it isDVSA fee (weekday)
Part 1aTheory: multiple-choice£26
Part 1bTheory: hazard perception£11
Part 2Case studies£23
Part 3aOff-road exercises£40
Part 3bOn-road driving£115 (£141 evenings/weekends)
Part 4Practical demonstration£55 (£63 evenings/weekends)

The fees are the same for all lorry and bus categories. If you fail, you pay the full fee again to rebook. Parts 2 and 4 are the extra CPC tests for professional drivers. Read more in Driver CPC explained.

Step 4: Train with a driving school

Most new drivers take lessons with an HGV training school before parts 3a and 3b. Training prices are set by schools. See costs and funding, including free Skills Bootcamps.

Step 5: Stay qualified

Complete 35 hours of periodic Driver CPC training every 5 years. Lorry and bus licences are renewed every 5 years. From age 45 each renewal needs a D4 medical, and from 65 you renew every year with a medical.
